The Dalglish Family Hearts & Minds Clinic for 22q11.2 Deletion Syndrome

Transition Clinic

A number of patients from the 22q Deletion Syndrome Clinic at the Hospital for Sick Children are reaching adulthood. They will have to say goodbye to the pediatric doctors who have been taking care of them since they were very young. From this point on, they will have appointments with doctors who take care of adults.

To make this move easier, the Dalglish Family 22q Clinic, in collaboration with the SickKids Good 2 Go Transition Program and the 22 Deletion Syndrome Clinic, offers the 22q Transition Clinic several times a year. The next Clinic takes place on Tuesday, July 18^th, 2017. Five teenagers, along with their parents and/or caregivers, will learn about the transition from SickKids to Adult Care at our Clinic.
